We pride ourselves on choosing the best treatment option for you. Please arrive 15 minutes early to allow time to register as a new patient.

You will receive the most out of your first appointment, by preparing ahead of time. In preparation for your first appointment, please make sure to complete the attached documentation:

Patient Registration: In order to receive the most out of your first appointment, we highly recommend that the following documentation will be completed.

Records Release (optional): If you would like us to obtain copies of your medical records from another office, please complete the record's release in full by filling out the last page listed in the patient registration form.

Remember to bring the following to your first appointment:

Driver’s license or picture ID

Completed paperwork

Insurance card

Police report (if applicable)

Attorney information

Claim number (if applicable)

List of current medications

Name of hospital

Please make sure to check with your insurance company BEFORE your appointment to be informed of your coverage (co-pays, deductibles, coverage, etc.). Every insurance plan/case is different and while we may contract with your overall insurance provider, your individual plan coverage will vary based on the policy you have.
